Application forms for the licensing of Chinese medicines traders and the registration of proprietary Chinese medicines, etc. can be obtained from and, after completion, should be returned to the Chinese Medicine Regulatory Office of the Department of Health.

Note: Except Payment for General Demand Note; the Chinese Medicine Regulatory Office of the Department of Health will issue General Demand Note in respect of the various kinds of fees related to the registration of proprietary Chinese medicines and the licensing of Chinese medicine traders. Payers should make payment according to the payment methods stated in the General Demand Notes.
